Physicals Examinations are required for Kindergarten and Fifth grade entry, and for students entering school for the first time. Physicals must be completed no earlier than 1 year prior to the student’s first day. Students transferring from out-of-district should contact the nurse’s office regarding physical requirements. Students not meeting the physical examination requirement are subject to exclusion from school. Download a School Physical Form here.
Immunizations are required for school attendance. For requirements specific to each grade level, please call the nurse’s office, or visit http://www.idph.state.il.us/about/shots.htm Medical and religious objections to any immunizations must be submitted in writing.
Dental Examinations are required for kindergarten, second and sixth grade students. Proof of exam must be on file in the nurse’s office by May 15 th of the school year. The exam must be completed by a licensed dentist and performed within 18 months of the May 15 deadline. The Dental Form may be downloaded here
Medications: In order for students to receive any medication at school, an order form completed by the doctor and signed by a parent must be on file in the health office. Medication is to be kept in the nurse’s office with the exception of students who self-carry medication for asthma, or emergency medication such as Epi-Pen. Asthma: The incidence of asthma has been increasing among school-age children. A medication order is required for any medication to be given at school. For more information about asthma, visit: . http://www.aafa.org/display.cfm?id=8 . An Asthma Action Plan may be downloaded here.
Food Allergies: We have an increasing number of students with food allergies attending school in District 200. Please contact the nurse’s office if your child has a food allergy that may require emergency attention. Communicable Diseases: Contact the health office if your student has or is being treated for strep throat, chicken pox, rotavirus, hand, foot and mouth disease or Fifth’s disease.
Vision and Hearing Screenings are conducted yearly by the DuPage County Health Department. Vision and hearing screenings are done for special education students, kindergarteners second graders and students new to the district. Hearing screenings are conducted on first and third graders. District nurses will conduct vision screening on fourth graders. Please contact the nurse if you have any concerns regarding your child’s vision or hearing.
